Important Information
 

For Visiting Students to the South Carolina College of Pharmacy MUSC Campus:
 Complete 4 weeks in advance of rotation!  Paperwork will only be processed after enrollment at an accredited College/School of Pharmacy has been verified once a letter of good standing has been received.
STEP 1 - Student Health Clearance

bullet MUSC Immunization Verification for Visiting Students (complete with appropriate signatures)
Visiting students must be cleared through MUSC's Student Health prior to the start of their rotation
bullet USC Students ONLY - Health Records Release (if needed to release records to MUSC)
Non-USC students contact your school's student health for records release form
bulletStudent must also submit a copy of their health insurance card.

STEP 2 - Human Resources Forms

bullet Non-paid Opportunity Form
bulletInformation obtained from these forms will be used to complete a Level-One Criminal Background Check which includes the following:
bullet Criminal History Investigation (7 years)
bullet Sexual Offender Registry / Predator Registry
bullet Social Security Number Verification
bullet Positive Identification National Locator with Previous Address
bullet Maiden/ aka Name Search
bullet Medicare/Medicaid Sanctioned, Excluded Individuals Report
bullet Office of Research Integrity Search
bullet Office of Regulatory Affairs Search
bullet FDA Debarment Check
bullet National Wants and Warrants Submission
bullet Investigative Application Review
bullet National Healthcare Data Bank Sanction Report
bullet Misconduct Registry
bullet EO 13224 Terrorism Sanction Regulations

STEP 3 - Student Training

bullet OSHA online Training (print completion letter when finished)
bullet OACIS (for SCCP USC Campus students ONLY)
bullet OACIS Non-MUSC Sponsor Form & Patient Privacy Policy
           this also includes HIPAA Training on pages 2-6
(Acronym: Open Architecture Clinical Information System - online medical records access)

STEP 4 - Temporary Student ID

bulletClearance will be given by Human Resources for Public Safety to issue a temporary ID once everything has been completed
